The East Bay Community Care Network (EBCCN) is a multi-service, community-driven organization committed to enhancing the health and well-being of individuals and families across the East Bay. Through collaborative care models and innovative programs, EBCCN addresses the interconnected challenges faced by our most vulnerable populations.

Purpose Statement

EBCCN exists to reduce barriers to care, uplift community resilience, and improve quality of life by offering wraparound services that promote physical health, mental wellness, social stability, and the human-animal bond.

EBCCN is a coalition of healthcare professionals, case managers, peer advocates, harm reduction specialists, and animal welfare advocates working together under a unified mission. With deep roots in the East Bay, our team reflects the communities we serve, bringing lived experience, cultural humility, and unwavering commitment to justice and healing. 

Who We Do
EBCCN provides a comprehensive continuum of services that include:

1. Case Management & Intensive Case Management: Supporting individuals with complex medical, behavioral, and social needs through personalized care planning, system navigation, and advocacy.

2. Patient Care Coordination: Ensuring seamless connections between healthcare providers, mental health services, housing, and social supports.

3. Harm Reduction & Substance Use Treatment/Prevention: Delivering evidence-based interventions, peer support, overdose prevention, and treatment referrals within a non-judgmental, client-centered framework.

4. Reproductive Health Services: Offering inclusive sexual and reproductive health education, STI prevention/treatment, contraceptive care, and pregnancy options counseling.

5. Pet Rescue Services (Purrific Rescue): Providing rescue, adoption, and care support for companion animals, especially those belonging to individuals experiencing housing insecurity or crisis.
