At East Bay Community Care Network (EBCCN), we believe every person deserves dignity, health, and safety. Our services are designed to meet people where they are, whether on the street, in shelters, schools, colleges, or community centers, and provide trauma-informed, culturally responsive, and equity-rooted care.
We focus on housing, behavioral health, harm reduction, youth empowerment, and family support. All services are low-barrier, confidential, and free of charge.
Core Programs
Our Case Management Program provides personalized, trauma-informed support to help clients navigate complex systems and achieve stability.
What We Offer
Intensive Case Management for unhoused or high-acuity individuals
General Case Management for youth, families, and marginally housed clients
Patient Care Coordination for medically fragile clients or college students
Housing navigation and retention support
Behavioral health coordination and harm reduction planning
Reproductive and sexual health education and access
School IEP assistance, family reunification, and legal referrals
Help applying for benefits (SSI, SSDI, Medi-Cal, CalFresh, GA, CalWORKs, Covered CA)
Peer mentorship and culturally affirming navigation
Crisis response, safety planning, and emotional support
Services are delivered in-home, in the field, on campus, or virtually, wherever clients feel safest.
Harm Reduction & Overdose Prevention
We believe every life is valuable and that compassion must be at the center of our response to substance use. Our harm reduction program uses evidence-based strategies to reduce overdose deaths and promote safety without judgment.

Naloxone (Narcan) distribution & training
Fentanyl test strips
Sterile syringes & safer use supplies
Sharps containers for safe disposal
Condoms & safer sex kits
Wound care & hygiene kits (bandages, wipes, menstrual supplies)
Peer-led education, overdose prevention training, and referrals
Our Services Include
Weekly street outreach with naloxone distribution and overdose response training
Mobile harm reduction vans bringing supplies to underserved neighborhoods
Substance use safety workshops in schools, colleges, and universities
Harm reduction saves lives today. Abstinence is never a requirement for care.

Housing & Homelessness Support
Navigation of shelter, transitional, and permanent housing systems; help with documentation and verifications.
Behavioral Health & Crisis Support
Warm handoffs to mental health/SUD services, CARE Court navigation, conservatorship support, and crisis stabilization referrals.
Benefits Enrollment & Systems Navigation
Assistance applying for and maintaining Medi-Cal, CalFresh, SSI/SSDI, GA, CalWORKs, and Covered CA.
Family & Youth Support
School advocacy, trauma-informed counseling referrals, caregiver coaching, and peer mentorship on high school and college campuses.
Reproductive Health Access
Confidential education and referrals for contraception, STI testing/treatment, and pregnancy-related care.
Street Medicine Coordination
Referrals to medical teams for wound care, MAT (medication-assisted treatment), vaccines, and follow-up support.
